1. Acceptance of Terms
By registering and using the PDM Health Services Office Student Health Portal (STEALTH), you acknowledge that you have read, understood, and agree to be legally bound by these Terms and Conditions. If you do not agree, you must not proceed with registration or use of this portal.
2. Eligibility and Authorized Use
Access to STEALTH is strictly limited to currently enrolled students of Pambayang Dalubhasaan ng Marilao (PDM). Unauthorized access, use by non-enrolled individuals, or use for purposes other than those stated herein is strictly prohibited and may be subject to disciplinary and/or legal action under applicable laws of the Republic of the Philippines.
3. Account Responsibility and Security
You are solely responsible for maintaining the confidentiality and security of your account credentials. You must not share your student number, email address, password, or any access information with any other person. Any activity conducted through your account will be considered your own. In the event of suspected unauthorized access or breach of account security, you must immediately notify the PDM Health Services Office. The institution shall not be held liable for any loss or damage arising from your failure to safeguard your credentials.
4. Medical Emergency Consent
As required by PDM, you are obligated to submit a duly accomplished Medical Emergency Treatment Consent Form through this portal. By submitting this form, you grant PDM Health Services Office and authorized medical personnel the consent to administer necessary emergency medical intervention in the event that you are incapacitated and unable to provide consent at the time of a medical emergency. This consent is valid for the duration of your enrollment unless formally revoked in writing.
5. Daily Symptom Reporting
You are permitted to submit one (1) symptom report per day. Submitting multiple reports within the same day, submitting false or fabricated symptoms, or using the reporting feature for any purpose other than genuine health disclosure is strictly prohibited.
6. Honesty and Integrity in Health Reporting
You are legally and ethically obligated to provide truthful, accurate, and complete information in all health reports and submissions made through this portal. Deliberate submission of false, misleading, or exaggerated health information constitutes a violation of institutional policy and may be considered a breach of the school's Code of Conduct. Repeated false reporting or abuse of the symptom reporting system may result in disciplinary proceedings before the Student Disciplinary Office (SDO).
7. Privacy and Data Confidentiality
All personal and health information collected through STEALTH is treated as strictly confidential and is protected under Republic Act No. 10173, known as the Data Privacy Act of 2012. Your data will not be disclosed, sold, or transferred to any third party without your explicit consent, except as required by law or as necessary for the delivery of health services within the institution. You have the right to access, correct, and request the deletion of your personal data in accordance with applicable law.
8. Prohibited Acts and System Misuse
The following acts are strictly prohibited and may constitute a violation under Republic Act No. 10175 (Cybercrime Prevention Act of 2012):
• Unauthorized access to another student's account or health records;
• Tampering, altering, or attempting to manipulate health data within the system;
• Sharing, publishing, or exposing another user's health information without consent;
• Using automated tools, scripts, or bots to interact with or manipulate the portal;
• Any act intended to disrupt, degrade, or compromise the integrity of the system.
9. Disciplinary Consequences
Any violation of these Terms and Conditions may result in the immediate suspension or permanent termination of your portal access, referral to the Student Disciplinary Office (SDO) for formal proceedings, and/or referral to appropriate legal authorities depending on the severity of the violation. Sanctions shall be imposed in accordance with the PDM Student Handbook and applicable Philippine laws.
10. Limitation of Liability
PDM Health Services Office shall not be held liable for any medical outcomes arising from incomplete, inaccurate, or delayed information provided by the student. The portal is a supplementary health management tool and does not replace professional medical consultation or emergency services.
11. Amendments to Terms
PDM Health Services Office reserves the right to amend, update, or revise these Terms and Conditions at any time without prior notice. It is your responsibility to review these terms periodically. Continued use of the portal following any modifications constitutes your acceptance of the revised terms.
12. Governing Law
These Terms and Conditions shall be governed by and construed in accordance with the laws of the Republic of the Philippines. Any disputes arising from the use of this portal shall be subject to the jurisdiction of the appropriate courts or administrative bodies within the Philippines.
